Job summary

VyTalent Solutions is seeking a Home Health RN for a travel contract in Colmar, PA. The role offers an 8am-5pm schedule, 13-week length, and a local/traveler-friendly candidate type. You will provide nursing care, perform assessments, and manage chronic conditions across a geographic area.

Documentation will be done in systems like EPIC, HCHB, OASIS, and Meditech. Collaboration with interdisciplinary teams and patient education are key parts of the position.

Qualifications

• RN license and active state registration.
• Experience in home health or similar settings preferred.
• Ability to perform comprehensive patient assessments and manage chronic conditions.

Responsibilities

• Provide comprehensive nursing care to home health patients in a defined area.
• Conduct patient assessments, wound care, and medication administration for chronic conditions such as CHF/COPD/diabetes.
• Document treatments accurately using EPIC, HCHB, OASIS, and Meditech.
• Educate patients and caregivers on health management and safety protocols.
• Collaborate with interdisciplinary teams to develop and implement care plans.
