Job summary

Compassus is seeking a Home Health Registered Nurse - Triage to handle after-hours calls from patients and facilities, triage referrals, and support the field staff. The role emphasizes coordination with physicians and telehealth oversight, with weekend shifts on Saturdays and Sundays, 9:00 AM–7:00 PM Central Time.

Responsibilities include facilitating referrals, educating referral sources and families, and arranging admissions and routine visits while maintaining high-quality patient care.

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Two years of full-time RN experience or two years of clinical home health experience.
• Active Compact RN license in state(s) of employment; CPR certification required.

Position Summary

Remote. The Home Health Registered Nurse - Triage triages all patient and facility calls after hours and may include facilitating new and existing home health referrals from in and out of system, after hours, and support for home health field staff. Potential Schedule: Every Saturday and Sunday, 9:00 AM-7:00 PM Central Time.

Position Specific Responsibilities

• Accepts and facilitates new referrals for both in and out of the system for home health and hospice patients.
• Acts as a liaison to provide community education for our referral sources, patients and families.
• Provides accurate and responsive service to all calls received during weekend hours.
• Collaborates with physicians and obtains orders as needed to facilitate patient care.
• Provides guidance and support to field staff during hours worked.
• Assists in scheduling patient admissions, routine visits, and PRN visits to field staff (Nurses, Therapists, and Home Health Aides).
• Monitors telehealth as needed.
• Performs other duties as assigned.

Education and/or Experience

• Associate or Bachelor's degree in Nursing required.
• One (1) year of experience as a registered nurse in a clinical care setting or home health preferred.

Skills

• Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
• Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.

Certifications, Licenses, and Registrations

• Active and unencumbered Compact Registered Nurse license in state(s) of employment required.
• CA, OR, MI, RN Licensure required. Able and willing to obtain additional licensures.
• Current CPR certification required.

State Specific Requirements

• Two (2) years of full time experience as a Registered Nurse; or two (2) years of clinical experience in home health required.

Physical Demands and Work Environment

The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
