Job summary

CVHCare, a leader in Home Health Clinical services, is seeking a Registered Nurse for weekend field visits in the California home health region. This full-time role involves managing patient health plans, wound therapy, post-surgical care, and patient education in patients' homes.

The RN will operate as a case manager, collaborating with primary care physicians, with flexible weekend schedules (Thursday-Monday variants) depending on area coverage. CA RN license and CPR/BLS required.

Qualifications

• Minimum 1 year RN experience in acute, SNF, home health or hospice setting
• Active California Registered Nurse license
• AA, BSN, MSN degrees
• Reliable transportation and CPR or BLS certification

Responsibilities

• Monitor patient health and develop an individualized plan of care with a physician
• Serve as case manager and deliver in-home care independently
• Provide education on disease management and wound care (including wound therapy, post-surgical, ostomy)
• Schedule visits around patient needs and maintain flexibility for travel within area

Title:Weekend Home Health RN (Field)
Schedule: Full Time (24 visits per week w/benefits)
* Non-traditional schedules are available, i.e., Thursday-Monday
Locations: Area #4 covers the following cities:Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek and Area #5 covers the cites:Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, Bay Point
Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm onsite in San Ramon

Position Summary:The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management.

We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating the medication reconciliation, tracking down DME orders, getting MD signatures, coding, lab follow-up, etc., truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home.

Requirements:

• Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting
• Must Possess an active CA Registered Nursing License
• AA, BSN, MSN
• Reliable Transportation
• Current CPR or BLS Card
