Job summary

Advanced Nursing Concepts is seeking a licensed RN to provide one-on-one home health care across multiple Williston area counties. You will assess patients, develop and update care plans, administer medications, and educate families on managing care at home.

The role requires regular travel by personal vehicle; current license, CPR certification, and excellent communication are essential. This position offers a W-2 status with a flexible PRN schedule and mileage reimbursement.

Qualifications

• Graduate of an accredited school of nursing.
• Current state licensure and CPR certification.
• Bachelor’s degree preferred, with 1 year home health care experience.
• Excellent observation, verbal and written communication skills, problem solving, and nursing skills per competency checklist.

Responsibilities

• Assess each new patient’s condition, history, and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient’s status changes.
• Administer medications and treatments as prescribed.
• Educate patients and families on home care management.
• Coordinate with physicians and send updates at least every 60 days.
• Collaborate with community health partners involved in patient care.
• Plan and carry out discharge when the patient is ready.
• Arrange equipment or services the patient needs.
• Check in on home health aide care every two weeks.

• Schedule: PRN, successful candidates will have 2-3 days of availability weekly from 7:00a-4:00p

• Mileage: Currently $0.60/mile
• Reports to: Branch Director

What You'll Do

• Assess each new patient's condition, history, and home care needs

Qualifications

• Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
• Current licensure in state and CPR certification.
• Preferred: Bachelor's degree, with one (1) year of home health care experience.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• This position requires regular travel by personal vehicle between patient homes, the office, and other locations throughout the assigned service area. The clinician must hold a valid driver's license, maintain reliable transportation and current auto insurance, and be able to drive safely for extended periods in varying weather and traffic conditions.

Physical Demands

This role requires the following, with or without reasonable accommodation:

• Prolonged walking and standing
• Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally
• Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)
• Reaching, stooping, bending, kneeling, and crouching
• Visual acuity and hearing sufficient to perform nursing duties

Health Requirements

Required before providing patient care:

• Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
• Documented negative TB test (TST or TB blood test) taken within the past 12 months
• TB risk assessment and symptom evaluation upon hire
