Advanced Nursing Concepts provides one-on-one home health care to patients in their homes. We support our clinicians with field mentorship, clinical guidance, and flexible scheduling, so clinicians get the independence of home health without working in isolation.

Formación

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience in an institutional setting.
• Current licensure in state and CPR certification.
• Bachelor's degree preferred with 1 year of home health care experience.

Responsabilidades

• Assess each new patient's condition, history, and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient's status changes.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and send updates at least every 60 days.
• Coordinate with community health partners involved in the patient's care.
• Plan and carry out discharge when the patient is ready.
• Arrange any equipment or services the patient needs.
• Check in on home health aide care every two weeks.

Quick Facts

• Pay: Negotiable based on experience, knowledge, skills, and abilities

• Schedule: PRN, successful candidates will have 2-3 days of availability weekly from 7:00a-4:00p

• Mileage: Currently $0.60/mile

What You'll Do

• Assess each new patient's condition, history, and home care needs


• Build the care plan with the patient and family

Qualifications

• Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.


• Current licensure in state and CPR certification.


• Preferred: Bachelor's degree, with one (1) year of home health care experience.


• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.


• This position requires regular travel by personal vehicle between patient homes, the office, and other locations throughout the assigned service area. The clinician must hold a valid driver's license, maintain reliable transportation and current auto insurance, and be able to drive safely for extended periods in varying weather and traffic conditions.

Physical Demands

This role requires the following, with or without reasonable accommodation:

• Prolonged walking and standing


• Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally


• Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)


• Reaching, stooping, bending, kneeling, and crouching


• Visual acuity and hearing sufficient to perform nursing duties

Health Requirements

Required before providing patient care:

• Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease


• Documented negative TB test (TST or TB blood test) taken within the past 12 months


• TB risk assessment and symptom evaluation upon hire
