Description

Summary of Essential Functions for the Position:

Committed to Caring, a Registered Nurse (RN) administers skilled nursing care to patients in their place of residence on an intermittent basis. This is performed according to physician orders and the plan of care under the supervision of the Executive Director. The RN participates in patient, caregiver, and family counseling and education, communicates with all physicians involved, and provides services as ordered. The RN also contributes to the QAPI program, participates in interdisciplinary assessments, and collaborates in developing and revising the plan of care with patients and caregivers. Supervises care provided by LVNs and HHAs.

Qualifications

• Licensed in the practicing state(s)
• Minimum two years of experience, preferably in Home Health
• Proficiency with EMR systems (Kinnser/WellSky preferred)
• CPR certification from AHA or ARC
• Ability to exercise initiative and independent judgment
• Excellent communication and interpersonal skills
• Strong customer service orientation
• Supportive of agency's core values and goals
• Valid driver’s license, auto insurance, reliable transportation
• On-call availability at least once per month
• Willingness to travel 100% as required

Educational Requirements

• High School Diploma or GED; graduate of an approved nursing program

Working Conditions & Physical Requirements

• Work involves indoor office and patient home environments, requiring physical effort including lifting up to 100 pounds, and exposure to environmental elements and biological hazards
• Must have functional senses or adaptive devices to meet job demands
• Provide evidence of annual TB testing and other required examinations
