Job Overview

The Registered Nurse is responsible for providing and documenting skilled nursing care according to the care plan and physicians' orders, while maintaining confidentiality and professional boundaries.

Schedule and Territory

• Schedule: Monday - Friday, PRN/Part-Time visits needed
• Territory: Shelby County

Essential Job Functions

• Develop care plans with physician input, including medical interventions and measurable goals.
• Educate clients and families based on specific needs.
• Orient and train primary caregivers for optimal client functioning.
• Coordinate ongoing client care through documentation and communication with physicians and caregivers.
• Provide home care using skills such as phone triage, patient education, wound care, infusions, catheter care, PICC line dressing changes, etc.

Requirements

• Active RN license in the state of application
• Valid CPR certification

Preferred Qualifications

• Medicare Skilled Nursing experience
• Basic understanding of Oasis
• At least 1-year RN experience in a healthcare setting
