Centric Home Health and Hospice is seeking a compassionate Registered Nurse to provide in-home nursing care, coordinate patient care, and work with an interdisciplinary team to achieve optimal outcomes. The role involves travel to patient homes, executing skilled nursing procedures, and educating patients and caregivers.

The ideal candidate will have an RN license, 1+ year of clinical experience (home health preferred), CPR, and a valid driver’s license.

Qualifications

• Graduate of an accredited nursing program (diploma, ASN or BSN).
• Current RN license in the state.
• Minimum 1 year of clinical nursing experience; home health preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.

Responsabilités

• Conduct initial and ongoing patient assessments.
• Develop, implement, and update individualized plans of care.
• Provide direct skilled nursing care as ordered by physicians.
• Educate patients and caregivers on health needs and procedures.
• Communicate changes in patient condition to providers and care team.
• Complete clinical documentation accurately and timely.
• Participate in on-call rotation as assigned.
• Ensure compliance with infection control and agency policies.

Job Overview

We are seeking a compassionate and skilled Registered Nurse to provide quality in-home nursing care for patients. In this role, you will coordinate patient care, perform skilled nursing procedures, and work closely with an interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities

Minimum Requirements
