Job summary

One Home Health LLC is seeking an experienced Registered Nurse to join our home health team in Algonquin, IL. This role requires the ability to manage a caseload, perform skilled nursing visits, and work independently in patients’ homes.

The ideal candidate has an active IL RN license and 1–2 years of home health experience, with knowledge of OASIS documentation and Medicare guidelines. Reliable transportation and strong clinical judgment are essential.

Qualifications

• Active, unencumbered IL RN license is required.
• Minimum 1–2 years RN experience, home health preferred.
• Knowledge of OASIS documentation and Medicare home health guidelines is required.
• Comfortable working independently in patients' homes across the service area.
• Reliable transportation and valid driver's license are required.
• Strong clinical judgment and ability to prioritize a full caseload without close supervision.

Responsibilities

• Manage a caseload of home health patients, performing skilled visits, assessments and care plan updates in the field.
• Perform admissions, recertifications, and discharges with accurate OASIS documentation.
• Coordinate with physicians, therapists, and the inter-disciplinary team on changes in patient condition.
• Educate patients and family caregivers on disease management, medications and home safety.
• Respond to changes in patient status and escalate when needed.
• Maintain documentation meeting Medicare, state, and agency standards.

One Home Health is looking for an experienced Registered Nurse to join our home health team. This isn't an entry-level role. We need someone who already knows how to manage a caseload, think on their feet in a patient's home, and work independently without a nurse’s station down the hall.
