Responsibilities

• Complete admissions, assessments, routine visits, recertifications, and discharges.
• Develop and update individualized plans of care.
• Monitor patients’ conditions and communicate significant changes.
• Coordinate care with physicians, caregivers, and interdisciplinary team.
• Educate patients and families about medications, conditions, safety, and self-care.
• Complete accurate and timely clinical documentation.
• Help prevent hospitalizations through proactive nursing care.

What You’ll Do

• Complete admissions, assessments, routine visits, recertifications, and discharges

What You’ll Need

• Current, unrestricted Massachusetts RN license
• Previous home health nursing experience required
• Ability to work independently and manage visits efficiently
• Availability to commit to a consistent part-time schedule
• Valid driver’s license, reliable transportation, and current auto insurance
• Willingness to travel throughout Bellingham and surrounding communities
• Ability to sit, stand, bend, lift, and move intermittently and lift 50-100 pounds
