Job summary

Compassus is seeking a Home Health Registered Nurse (RN) to support patients at home, helping them recover, remain independent, and enjoy quality time with loved ones through flexible scheduling and a caring culture.

This role emphasizes patient and family education, case management, and clinical oversight across an interdisciplinary team, delivering compassionate, high-quality care within Ascension at Home and the Compassus network.

Qualifications

• RN license in the state you work.
• Two years of RN experience, home health preferred.
• Current driver’s license.
• CPR and BCLS certification.

Responsibilities

• Support patient recovery and ability to stay at home - enabling them to spend quality time with loved ones and doing the activities they enjoy.
• Educate patients and families to reach healthcare goals by disease management.
• Case manage and provide clinical oversight to interdisciplinary team.
