Job summary

Compassus at Ascension at Home, Together with Compassus, is seeking a Home Health Registered Nurse (RN) to support patient recovery at home and empower families to reach health goals.

You will educate patients on disease management, provide case management, and oversee the interdisciplinary team to ensure high-quality home-based care.

Qualifications

• RN license in the state you work
• Two years of experience as a registered nurse, home health experience preferred
• Current driver’s license
• Current CPR and BCLS certification

Responsibilities

• Support patient recovery and ability to stay at home—enabling them to spend quality time with their loved ones and doing the activities they enjoy
• Case manage and provide clinical oversight to interdisciplinary team
• Empower patients and families to reach healthcare goals by educating them on disease management

Home Health Registered Nurse (RN) Requirements
