Responsibilities

• Manage the Care Journey: Serve as the primary contact for a caseload of home health patients, coordinating services.
• Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans with the team.
• Coordinate Services: Collaborate with physicians, nurses, therapists, social workers, and aides to ensure timely care.
• Monitor and Evaluate: Track patient progress and update care plans; communicate with families.
• Documentation: Maintain accurate clinical documentation per Medicare guidelines and agency policy.
• Advocate for the Patient: Provide education and support to patients and families to promote independence.

What We’re Looking For

• License: RN (Registered Nurse) with active (state) license.
• Experience: At least 1 year of experience in home health or case management preferred.
• Skills: Strong critical thinking, communication, time management, and the ability to work independently.
• Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
• Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
