Humana, Inc. is seeking a professional home health nurse to join the patient’s treatment team, delivering skilled nursing visits in the patient’s home under physician-guided plans. Responsibilities include supervising Aide/LPN, coordinating with care teams, and ensuring HIPAA compliance.

Position requires an approved nursing program graduate and current Texas licensure, with at least one year of care experience; home health experience is preferred. Strong communication is essential.

Formación

• Graduate of an approved school of nursing.
• Currently licensed in the State of Texas.
• Minimum of one year current experience in health care community.
• Home health experience preferred.
• Excellent Verbal Communication.

Responsabilidades

• Provide initial and ongoing assessment of client needs using OASIS data elements.
• Formulate and implement plan of care with input from patient/caregiver and physician.
• Evaluate plan effectiveness and adjust as needed.
• Provide nursing supervision for LPNs and Home Health Aide as assigned.
• Document observations and treatments per agency policies.
