Home Care Providers of Texas seeks compassionate Full-Time Registered Nurses in the North Dallas TX area to provide skilled nursing in patients’ homes using modern tools. You will join a team that supports your schedule, with AI-driven Roger app to reduce charting time and HIPAA-compliant documentation.

Formación

• Active RN license (TX/Compact) required.
• CPR certification required.
• Home health experience in geriatric care preferred.
• Self-starter with strong organizational skills.

Responsabilidades

• Deliver skilled nursing care in the patient’s home per physician orders and care plan.
• Collaborate with interdisciplinary team to develop and update individualized plans of care.
• Administer nursing treatments and manage clinical activities for optimal outcomes.
• Monitor patient progress and communicate findings to care team, patient, family, physician, and case manager.
• Perform additional duties as needed.

Essential Job Functions

• Deliver Skilled Nursing Care - Provide high-quality, comprehensive nursing care in the patient's home in accordance with the physician’s orders and individualized plan of care.
• Collaborate on Plan of Care - Partner with the interdisciplinary care team to develop, implement, and update personalized care plans that address each patient’s unique needs.
• Manage Treatments and Interventions - Administer nursing treatments and manage clinical activities to support optimal health outcomes for each patient.
• Monitor and Evaluate Progress - Continuously assess patient progress and the effectiveness of services; communicate findings with the care team, patient, family, physician, and case manager.
• Additional duties as needed.

Requirements

• Active RN License (TX/Compact)
• Active CPR Certificate
• Home Health Experience – Prior experience ingeriatric home health care.
• Self-Starter with Organizational Skill

• Valid Driver’s License
