Job summary

Home Care Providers of Texas is seeking compassionate Full-Time Registered Nurses (RN) and clinicians for the North Dallas and Denton area. You’ll use the Roger app to streamline charting, reduce workload, and focus on patient care within a modern, AI-assisted platform.

Qualifications

• Active RN license in TX or Compact license.
• Active CPR certification.
• Geriatric home health care experience preferred.
• Valid driver’s license required.
• Self-starter with strong organizational skills.
• Compassionate care approach to patient needs.

Responsibilities

• Deliver skilled nursing care in patients' homes per physician orders.
• Collaborate with the care team to update personalized care plans.
• Administer nursing treatments and manage clinical activities.
• Monitor patient progress and communicate with patients, families and care team.
• Perform additional duties as needed.

Essential Job Functions

• Deliver Skilled Nursing Care - Provide high-quality, comprehensive nursing care in the patient's home in accordance with the physician’s orders and individualized plan of care.
• Collaborate on Plan of Care - Partner with the interdisciplinary care team to develop, implement, and update personalized care plans that address each patient’s unique needs.
• Manage Treatments and Interventions - Administer nursing treatments and manage clinical activities to support optimal health outcomes for each patient.
• Monitor and Evaluate Progress - Continuously assess patient progress and the effectiveness of services; communicate findings with the care team, patient, family, physician, and case manager.
• Additional duties as needed.

Requirements

• Active RN License (TX/Compact)
• Active CPR Certificate
• Home Health Experience – Prior experience ingeriatric home health care.
• Self-Starter with Organizational Skill
