Job summary

Providence at Home with Compassus is seeking a registered nurse to plan, implement, and oversee home health services for patients in their residence. You will work with an interdisciplinary team to deliver skilled nursing care and ensure comprehensive care plans.

Responsibilities include assessing patients, case management, documenting plans of care, and maintaining infection control practices. On-call coverage and strong communication with families and physicians are essential.

Qualifications

• Graduate of an accredited college/university with nursing degree.
• Current, unencumbered State License as a registered Nurse.
• Current Driver’s License.
• CPR certification and health clearances (TB/Hepatitis).

Responsibilities

• Assess home health patients to identify physical, psychosocial, and environmental needs.
• Case manages and provides clinical oversight to the interdisciplinary team and family.
• Implements and documents the plan of care and revises as necessary.
• Provide care with infection control measures.
• Supervise HHA/LVN to ensure quality and continuity of services.
• Ensure quality patient care with proper documentation.
• Monitor cases for third party payor compliance.
• Perform on-call duties as assigned.
• Communicate effectively with patient/family and health professionals.

Education/Licensing/Certification

Minimum Qualifications:

• Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing


• Current, unencumbered State License as a registered Nurse


• Current Driver’s License


• Current CPR, negative TB screen and Hepatitis consent/declination

Experience

• Two years of experience as a registered nurse in a clinical care setting, home health preferred.

Knowledge And Skills

• Nursing skills as defined as generally accepted standards of practice.


• Good interpersonal skills.

Compensation

The current compensation range for this position is $40.90– $63.49/hour. The specific rate of pay offered to a selected candidate will be determined based on relevant education, training, experience, geographic location, and other position-related factors. Compassus does not inquire about, consider, or rely upon an applicant's prior salary history in determining compensation. In addition to base compensation for the position, an employee working in this position may be eligible for pay differentials based on hours, shifts worked, or other criteria. They may also qualify for bonuses under applicable bonus programs or at Compassus’s discretion, as well as other forms of variable pay under relevant company policies.A summary of applicable pay programs will be provided during the offer process.
