Enhabit Home Health & Hospice is seeking a licensed professional to provide skilled care to patients in the Logan, Utah area. You will teach patients and families, follow physician orders, and implement the established plan of care with compassion and professionalism.

Must hold an active license in the state, have at least one year of licensed experience, and demonstrate basic tech skills including mobile device use. Valid driver’s license and reliable transportation required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsabilités

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
