Job summary

Enhabit Home Health & Hospice Careers is seeking a licensed professional to provide skilled care to patients in their homes in Idaho. You will educate patients and families and support the care team in executing the physician’s orders and the plan of care.

Must hold an active state license, have at least one year of relevant experience, and demonstrate basic technology skills including a mobile device. A valid driver license and reliable transportation are required for home visits.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year experience as a licensed professional.
• Basic technology skills, including operation of a mobile device.
• Medicare home health or hospice experience preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
