Job summary

Atrinity Home Health, part of LHC Group within the UnitedHealth Group family, seeks a Registered Nurse in Home Health to deliver in-home nursing care and coordinate a total plan of care with physicians and the healthcare team.

You will supervise home health aides, complete OASIS assessments, document visits, and collaborate across disciplines to ensure safe, compliant, and high-quality patient care in patients' homes.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid Driver's License and access to reliable transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in coordination with the physician and health care team.
• Supervises care by home health aides and licensed nurses, providing instruction and assignments per regulations.
• Completes comprehensive assessments (OASIS) and reconciles medications as required.
• Documents visits per policy and payer requirements; ensures timely communication with team, physicians, patients and families.
• Participates in on-call and weekend rotations; supports quality improvement and adherence to utilization management.
