Job summary

Atrinity Home Health, part of LHC Group and UnitedHealth Group, seeks a Registered Nurse to provide in-home nursing care per physician orders and agency policies. You will coordinate the plan of care with the care team and families to ensure continuity and quality of care.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to dependable transportation.
• Current CPR Certification or ability to obtain within 90 days of hire.
• Ability to work with diverse patient populations.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws and agency policies.
• Coordinate total plan of care with other health professionals and family to maintain continuity of patient care.
• Initiate, develop, implement, and revise the plan of care with the physician and team.
• Supervise home health aides and licensed nurses, provide instruction, and assign tasks per regulations.
• Complete supervisory visits and ensure high-quality clinical services and infection control.
• Document visits, conduct comprehensive assessments, and communicate with physicians and caregivers to optimize outcomes.
