Responsibilities

What You’ll Do:

Conduct patient assessments and create individualized care plans

Deliver top-quality clinical care and monitor progress

Collaborate with LPNs, therapists, and home health aides to ensure holistic care

Educate patients and families on health conditions and care techniques

Provide compassionate, patient-centered nursing in a home health setting

Qualifications

What You Bring:

Active RN license in Pennsylvania

Minimum of one year of nursing experience (home health preferred, not required)

A commitment to quality, compassion, and clinical excellence

Strong communication and critical thinking skills
