Home Health Registered Nurse Case Manager (part time)

Job Overview

We are seeking a compassionate and dedicated Home Health Registered Nurse Case Manager to join our team on a part-time basis. In this role, you will provide telephonic care management and support to patients in Allentown, PA, and surrounding counties. This position is initially for Per-Diem with potential to transition to part-time or full-time.

Responsibilities

• Review patient assessments and develop individualized care plans with the care team.
• Ensure all skilled nursing care, including wound care, medication management, and IV therapy, is provided as ordered.
• Coordinate and communicate with healthcare team members to ensure comprehensive and effective care.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on proper care techniques and self-management.
• Document transfer and discharge Oasis as required.

Minimum Requirements

• Valid RN license in Pennsylvania.
• At least 1 year of experience in home health or a similar setting.
• At least 1 year of experience in case management.
• Strong clinical and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation.
