Summary:

Responsible for partnering with the physician and multidisciplinary team in coordinating patient care with the objective of enhancing quality and cost-effective care. Acts as a resource and patient advocate to facilitate safe transitions across the continuum.

Position Responsibilities:
  1. Care Coordination: Completes assessments and plans to meet patient needs, monitors length of stay, promotes resource utilization, and maintains current knowledge of clinical guidelines. Engages physicians to ensure seamless continuity of care and acts as a consistent resource. Resolves problems, communicates with patients and families, identifies and escalates issues impeding progress, and refers cases for social work intervention as needed.
  2. Documentation: Ensures complete and accurate documentation of care plans in EMR and case management systems.
  3. Metrics: Meets specific goals, objectives, and dashboards contributing to organizational success.
  4. Organizational Participation: Engages in improvement activities, patient satisfaction initiatives, Six Sigma committees, departmental and community activities.
  5. Compliance: Understands and applies federal and state requirements, reports compliance issues appropriately.
Position Qualifications Required:

Experience: Minimum of 3 years clinical nursing (RN) experience or social work; at least 1 year of case management or interdisciplinary experience in acute care, behavioral health, or related settings, with understanding of Medicare, Medicaid, managed care, and discharge planning (3-5 years preferred).

Education: RN or Masters prepared social worker.

Training/Certification/Licensure: RN or LSW required; LCSW considered.
