Visiting Nurse Association Of The Wabash Valley Inc in Terre Haute, IN is seeking a Home Health RN Case Manager to oversee patient care plans and coordinate with a team of aides and nurses. Qualifications

Responsibilities

• Oversee implementation and ongoing assessment of the patient’s plan of care through management of home health aides, LPNs, RNs, and other caregivers.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with appropriate providers to adjust care.
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care.
