Job summary

Ascension at Home together with Compassus is hiring a Home Health Registered Nurse (RN) to support patients at home, providing clinical oversight and education to families to manage diseases.

You will coordinate care as part of an interdisciplinary team, ensuring high-quality patient outcomes while promoting independence and safety in the home setting.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, home health experience preferred.
• Current CPR and BCLS certification.
• Current driver’s license

Responsibilities

• Support patient recovery and enable them to remain at home with their loved ones.
• Educate patients and families to reach healthcare goals and manage diseases.
• Provide case management and clinical oversight to the interdisciplinary team.
