Job summary

Legacy Lifecare VNA/Hospice is seeking a Home Health Registered Nurse to deliver compassionate care in patients' homes. This full-time role involves performing routine assessments, monitoring vitals, administering treatments, and coordinating with the care team to support palliative and hospice needs.

The position offers a Monday–Friday schedule, 40 hours per week, with occasional on-call rotations. MA RN license and at least one year of RN experience in home health or hospice are required.

Qualifications

• Current, unencumbered MA RN License.
• Minimum 1-year RN experience.
• Prior experience in home-care, hospice, rehabilitation, or long-term care setting preferred.
• Reliable transportation and ability to travel to patient homes or hospice facilities.
• Ability to communicate fluently in English – both verbally and written.

Responsibilities

• Provide kind, compassionate care to residents in home settings.
• Observe residents and report changes in condition or response to treatment.
• Perform basic resident care tasks including monitoring temperature and blood pressure.
• Support palliative and hospice care needs, including pain management and symptom control.
