Job summary

Domnovate is seeking an experienced Registered Nurse to join our home-health team in Le Mars, IA. This 13-week W-2 assignment offers day shifts (Mon-Fri) with 32–40 hours per week and a 1-in-4 call rotation that includes nights, weekends, and holidays.

You will perform OASIS assessments, develop individualized care plans, educate patients and families, and coordinate care across the team as you travel between patient homes in Plymouth County.

Qualifications

• RN license authorized to practice in Iowa.
• At least three years of relevant nursing experience, including two years performing OASIS assessments and home-care planning.
• Ability to work independently and communicate with patients, families and the care team.
• Epic experience is a plus.

Total Compensation: $37.85–$40.35/hour*

Support people across Plymouth County with thoughtful assessments, individualized care plans and clear education that helps patients and families feel more confident between visits. This assignment combines independent clinical judgment with the continuity of home-health care, giving you the chance to follow progress and build meaningful connections over time.

Assignment Overview

• Le Mars, Iowa 51031
• 13-week W-2 contract; 1 opening
• Day shifts, Monday-Friday; the facility considers 32-40 hours per week.
• 32 guaranteed hours per week; the hourly-equivalent compensation estimate uses a 40-hour planning week.
• One-in-four call rotation covering nights, weekends and holidays; standby and callback compensation is additional and confirmed before an offer.
• Travel between patient homes in Plymouth County is part of the role.
• Proposed start: October 28, 2026, pending facility confirmation.

What You Bring

• RN license authorizing practice in Iowa by assignment acceptance.

Pay and Benefits

Taxable W-2 hourly pay: $20.00–$22.50.
