Mary Free Bed Rehabilitation Hospital is seeking a Home Health Registered Nurse to provide skilled nursing care to patients in Grand Haven, MI and surrounding areas. A $10,000 first-year bonus is offered for achieving key milestones in the first year.

Qualifications

• Active RN license valid in the state you work.
• One year of RN experience delivering patient care.
• Valid driver’s license and ability to drive to patient locations ~20% of workday

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
