Job summary

HealthCare California is seeking a Home Health Registered Nurse in Fresno, CA. The role provides skilled nursing care at patients' residences, with case management of nursing and non-skilled care as assigned. All services align with state laws and standards.

You will assess patient needs, perform nursing care per the plan of care, and support treatments in a home setting while maintaining compliant documentation and flexible scheduling.

Qualifications

• Valid California Registered Nurse license.
• A minimum of 2 years of experience in a Home Health or Acute Care nursing setting.
• Current CPR certification.
• Valid California Driver's License.
• Computer skills, including typing for reporting and updating documentation in an online portal.
• Willingness and ability to work with electronic input and telecommunication devices.
• Excellent clinical skills and demonstrated ability to manage time well.
• Excellent communication skills; able to communicate clearly with staff and the public, verbally and in writing.

Responsibilities

• Initiate and identify patient needs through assessment.
• Provide nursing services based on the plan of care.
• Prepare necessary items for treatments and procedures.
• Implement and follow current standards to prevent infection; prepare medications as needed.
• Make treatment recommendations based on patient acuity and goals.
• Excel at the admissions process, including same-day electronic documentation.
• Remain flexible to accommodate admissions within time, travel, or geographic constraints.
• Provide and document skilled care on all visits, meeting agency documentation standards.
• Drive throughout the day to visit patients in their homes.

Job Description:
This position is responsible for providing skilled nursing care and services to patients in their place of residence, and may case manage nursing and non-skilled care and services when assigned. All nursing services are performed in compliance with state and local laws/regulations, the Nurse Practice Act, and all other healthcare accreditation standards.
