Job summary

Creative Solutions Services, LLC seeks an RN - Home Health for a 13-week onsite contract in Charlotte, NC 28262. Typical day includes Day Shift (8 hours, 5 days/week) with 6 patient visits and travel between University City, Cornelius, and nearby towns.

Required: active RN license, 1 year home health, BLS, valid driver’s license with insurance. Mileage reimbursed; scrubs navy; orientation provided.

Qualifications

• 1 year Home Health Nursing experience required.
• Driver's License and proof of Insurance; BLS.

Responsibilities

• Make visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and surrounding areas; 6 visits per day.
• Oversee/episode management for 25-30 patients overall.
• Schedule visits the day prior to planned visits; mileage is reimbursed weekly.

Job Description

Day Shift 8 hours/day 5 days/week

Title: RN - Home Health

Location: Charlotte North Carolina 28262

Duration: 13 Weeks

Shift: 8hr Day Shift - 40

Shift Information: Start time between 7-9

Clinical Interviewing: Yes – manager to conduct final interview

On Call, Call Back Requirements: Call is not required, but may be available if interested

Weekend Rotation: None, unless traveler is willing and interested in working weekends Unless posted for Weekend only Work

Holiday Requirements: Travelers will be expected to work all holidays, unless approval is received from Manager

Floating Requirements: N/A

• Home Health
• Radius rule: Traveler's permanent address must be 50 miles outside of home unit

Department Information

• Will make visits throughout University City, Cornelius, Davidson, Kannapolis, Salisbury, Concord, Huntersville, and cities in between
  • There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
  • Best central location for housing University City and/or Concord but must be comfortable to float throughout the areas above.
• Teammates are required to complete 6 visits a day (routine visits, discharge, ROC, recert, reassessment)

Minimum Years of Experience required: 1 year Home Health Nursing

• Will you accept a first-time traveler: Yes, if seasoned to home health

Patient Types/Common Diagnoses

• Chronic diseases, wound care/wound vac, IV, pleurx

Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance

• Charting experience with Homecare Homebase is required (no exceptions), IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS, case management (ability to oversee/episode management up to 25-30 patients at once)
