Description:This position involves providing coordinated home nursing care according to physicians' orders.You will be responsible for the competent application of the nursing process in home health settings.You will report to the clinical manager.The role includes direct supervision and evaluation of home health aides and licensed practical nurses.You must be willing to travel for home visits.Periodic office meetings will also be required.Reliable transportation, a current driver's license, and agency-required liability insurance are mandatory.You will be assigned to care for approximately 5-10 patients daily, which will account for productivity points.Each visit and activity, including documentation and travel, carries a different point value.You will need to achieve a minimum of 30 points per week.Types of cases include start of care, wound care, and follow-up visits.You must accept all patients on your schedule regardless of their proximity to each other.All documentation must be completed within 24 hours of each patient appointment.You should be comfortable with traveling up to 75 miles for patient visits.Mileage will be reimbursed at the standard rate.You will be required to participate in on-call rotations every 3-4 weeks.Weekend coverage may be necessary every 3-4 weeks, depending on location and needs.All travelers must have recent specialty experience.This position does not guarantee hours, and the clinician must meet weekly productivity expectations.If patient volume is low, there may be opportunities to float to another nearby clinic.You will be required to use HomeCare HomeBase and Oasis effectively during compliance.
