VNA of Ohio seeks an experienced RN Case Manager for its Home Health team in the Cleveland area. You will conduct comprehensive patient assessments, develop individualized care plans, and coordinate services with an interdisciplinary team to ensure high-quality care.

Responsibilities include home visits, progress monitoring, and education for patients and families. Opportunity offers a $5,000 sign-on bonus, top-quartile pay, and robust EMR/OASIS documentation support.

Qualifications

• Active RN license (Ohio) and current BLS.
• Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting.
• Proficiency in EMR systems and home health software.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized, evidence-based care plans.
• Coordinate services across an interdisciplinary team to ensure coordinated care.
• Perform home visits, monitor patient progress, and adjust care plans as needed.
• Educate patients and families on health management, medications, and available resources.
• Serve as the primary point of contact for patients, families, and referring providers.
• Maintain accurate EMR documentation including OASIS within required timeframes.
• Ensure compliance with Medicare, Medicaid, and agency regulations.
• Participate in team meetings and ongoing professional development.
