Job summary

Elara Caring is seeking a Physical Therapist Assistant to join our home-based care team in Connecticut. You will deliver direct physical therapy services under supervision, follow the Plan of Care, and ensure quality of care for patients in their homes.

You’ll travel within the branch area, educate patients and families on therapy home programs and assistive devices, and contribute to a collaborative, compassionate culture that prioritizes patient outcomes and caregiver support.

Qualifications

• Graduate of an accredited PTA program.
• Current, unrestricted PTA license valid for the state of work.
• 6+ months experience as a PTA preferred.
• Willingness to travel 50% within branch/office coverage area.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.

Responsibilities

• Provide direct physical therapy services under the supervision of a Physical Therapist to ensure quality and continuity of care.
• Follow Plan of Care as established by the Physical Therapist and report status changes promptly.
• Instruct patients and families in home exercise programs and use of prosthetic/orthotic devices.
• Travel between patient homes as required within the branch area.
• Document patient progress and maintain compliance with state practice guidelines.
