Job summary

Visiting Nurse Association of SEMO is seeking a licensed Missouri Physical Therapist Assistant to provide skilled physical therapy in patients' homes under physician orders.

You will work with the PT and care team to implement treatment plans, document progress, and coordinate care. Must have reliable transportation and pass standard background checks. Minimum 1 year experience preferred.

Qualifications

• Must hold a current Missouri PTA License.
• Minimum 1 year experience preferred.
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment checks.

Responsibilities

• Coordinate with PT and patient to develop a schedule meeting patient needs and physician orders.
• Provide direct therapy services per the plan and assess patient condition.
• Document patient responses to therapy and submit daily visit notes.
• Maintain documentation and care coordination records.
• Participate in patient care conferences and referrals as needed.
• Maintain inventory of supplies and ensure equipment cleanliness.
• Wear PPE as required.
