Qualifications

• Must be at least 18 years of age.
• Able to read, write, perform simple mathematics and follow directions.
• Must hold a current Missouri PTA License.
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening, criminal background check, registered w/Family Care Reg. and not be listed on the EDL
• Must complete an application in full and furnish at least 2 credible business references with correct addresses and telephone numbers
• Must be able to lift a minimum of 50# and sit for long periods of time
• Minimum of 1 year experience preferred

Responsibilities

• Works with PT and TCM and patient to develop a schedule that meets patient needs and physician orders.
• Develops a plan of treatment with the Physical Therapist and provides direct services per the plan, including assessment.
• Services provided in accordance with physician orders.
• Records and reports patient response to therapy to the PT.
• Maintains documentation and submits visit notes daily; documents care coordination.
• Participates in patient care conferences.
• Maintains an adequate inventory of supplies for therapy services; cleans and maintains equipment.
• Makes referrals for other disciplines as needed.
• Plans for supplies and monitors costs to minimize waste.
• Wears PPE as required.
