Job summary

Visiting Nurse Association of Southeast Missouri seeks a Physical Therapy Assistant to provide skilled services in patients' homes under orders from a physician. You will work with the PT and care team to implement treatment plans, document responses, and coordinate supplies and equipment.

A Missouri PTA license is required, along with reliability, transportation, and basic math and literacy skills. At least 1 year of experience is preferred.

Qualifications

• Must read, write and perform basic math.
• Must be able to follow directions and communicate clearly.
• Minimum of 18 years old and eligible to work.
• Must have Missouri PTA license.
• Must have reliable transportation and auto insurance.

Responsibilities

• Develop a schedule with PT and patient to meet needs and physician orders.
• Provide direct therapy services in home per treatment plan and assess patient conditions.
• Maintain documentation and submit visit notes daily; participate in care conferences.
• Plan for supplies, maintain equipment and coordinate care with other disciplines as needed.
