Job summary

Lorian Health is seeking a dedicated Physical Therapy Assistant to join our home health team in San Jose, CA. You will deliver skilled PT interventions under a licensed PT, helping patients improve mobility and independence in their homes.

Qualifications

• Graduate of a PTA program approved by the AMA/Association or equivalent.
• Licensed/registered to practice as a PTA in the state of practice.
• Current CPR certification is required.
• Valid driver’s license and insured personal vehicle per company requirements.
• Minimum of one year of Home Health or Hospice experience.

Responsibilities

• Provide skilled physical therapy interventions as ordered by the physician and under the direction and supervision of a licensed Physical Therapist.
• Support patients in improving mobility, balance, gait, strength, and functional independence.
• Educate patients and families/caregivers on home exercise programs, balance and gait training, and safety strategies.
• Monitor patient condition and progress, communicating changes with the supervising PT and care team.
• Assess the home environment and recommend modifications to promote safety and independence.
• Maintain accurate, complete, and timely clinical documentation.
• Collaborate with the interdisciplinary care team and participate in education and in-service programs.
