Responsibilities

• Promote independence and reduce reliance on medications through therapy
• Develop individualized care plans using proven techniques
• Create wellness programs tailored to each patient
• Educate and motivate patients to participate in rehabilitation
• Collaborate with healthcare professionals for coordinated care
• Document services accurately and maintain patient records
• Perform additional duties as assigned by supervisor
