Job summary

CenterWell Home Health is seeking a licensed Physical Therapist Assistant to work in patients' homes under the supervision of a Physical Therapist. You will implement the PT plan, perform therapeutic exercises, and support functional recovery in a home-care setting.

The role requires a valid state license, driver's license with reliable transportation, CPR certification, and teamwork with clinical staff. Pay per visit ranges from $41 to $57; full-time compensation ranges from $64,000 to $87,500

Qualifications

Active Physical Therapy license in the relevant state.
Valid driver's license with reliable transportation.
Current CPR certification.
Experience collaborating with a healthcare team.
Strong knowledge of geriatric physical, emotional and social changes.

Responsibilities

Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
Provide non-complex active and passive manual therapy, therapeutic exercises, and modalities as prescribed.
Document client records and submit clinical documentation weekly.
Attend educational meetings and coordinate department in-services.
Notify Physical Therapist or Branch Director of equipment or staff changes.
Apply infection control and universal precautions in patient care.
