Job summary

Frontier Home Health in the Fort Worth area seeks a Physical Therapist Assistant to deliver high-quality PT services under the supervision of a licensed Physical Therapist and the patient’s physician plan of care. The role focuses on evaluating and treating patients to relieve pain and restore function in a home health setting.

The PTA will guide and assist patients through therapeutic exercises, modalities, and mobility training, while documenting progress.

Qualifications

• Must hold active Texas PTA license and be licensed where the HHA is located.
• Graduated from a PT assistant program approved by CAPTE and passed national exam.
• Must possess and maintain current CPR certification; driving/transportation required.

Responsibilities

• Provides direct physical therapy under the direction of the licensed physical therapist and physician's plan of care.
• Treats patient to relieve pain and restore or maintain function and maximal performance.
• Guides patients in active and passive therapeutic exercises, gait and functional training, transfers, and prosthetic training.
• Utilizes physical agents such as heat, cold, water, light, electricity, ultrasound and massage.
• Observes patient response and reports changes in condition to the physical therapist and organization staff.
• Instructs patients on care and use of mobility aids and prosthetic/orthotic devices.
• Maintains clinical notes and records within prescribed time frames and integrates into patient chart.
• Participates in home health team meetings and in-service education.
