Job summary

Medi Home Health & Hospice in North Carolina is seeking a Full-Time Physical Therapist Assistant (PTA) to provide in-home physical therapy across Avery and Mitchell counties.

You'll work under a licensed Physical Therapist, perform mobility-focused treatments, document care, and educate patients and families; valid NC PTA license and driver’s license required; home health experience preferred.

Qualifications

• Completion of an accredited Physical Therapist Assistant program.
• Active PTA license in North Carolina.
• Valid driver’s license and reliable transportation.
• Minimum of 1 year of verifiable physical therapy experience.
• Previous home health experience preferred.
• Excellent communication, organization, and interpersonal skills.
• A strong commitment to compassionate, patient-centered care.

Responsibilities

• Deliver physical therapy services as outlined in the plan of care.
• Carry out treatment protocols that support mobility, strength, balance, and functional recovery.
• Monitor patient response to treatment and communicate updates to the PT or Case Manager.
• Maintain accurate, timely documentation in accordance with agency policy and regulatory requirements.
• Assist with educating patients and caregivers on exercises and mobility techniques.
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.
