Job summary

Snap Health in McKinney, TX is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health role. You will provide skilled PT services under the supervision of a Physical Therapist, helping patients improve mobility, strength, and functional independence.

You will perform therapeutic exercises, gait and balance training, monitor progress, educate patients and caregivers, document visits, and collaborate with the care team while adhering to agency policies.

Qualifications

Current Physical Therapist Assistant license in the State of Texas.
Minimum 1 year PTA experience preferred (home health experience a plus).
Strong clinical, communication, and time-management skills.
Ability to work independently in a home health setting.
Valid driver’s license, reliable transportation, and auto insurance.
Proficient with mobile apps and electronic documentation systems.

Responsibilities

Provide physical therapy treatments in accordance with the established plan of care.
Carry out therapeutic exercises, gait training, balance activities, and functional mobility training.
Monitor patient progress and report changes to the supervising Physical Therapist.
Educate patients and caregivers on exercises and safety techniques.
Maintain accurate and timely documentation of visits and patient progress.
Communicate effectively with the interdisciplinary care team.
Adhere to agency policies, procedures, and regulatory standards.
