Job summary

Amedisys, Inc. is seeking a Home Health Physical Therapist Assistant to provide therapy services at patients' homes. The role requires licensure as a PTA in the assigned state and CAPTE-accredited education, with a preference for candidates having 1 year of home health experience.

CPR certification is required. Responsibilities include delivering PT services under supervision, documenting patient progress, and contributing to care planning and quality activities while maintaining confidentiality

Qualifications

• Current license to practice as a PTA in the assigned state.
• Graduate from an accredited PTA program per CAPTE.
• One year PTA experience in home health settings preferred.
• If <1 year experience, mentoring and/or residency approval required.
• Current CPR certification.

Responsibilities

• Provide physical therapy services under the supervision of a PT and per plan of care.
• Observe and report patient response and changes to staff and physicians.
• Promote wellness and prevention strategies for patients.
• Document supervisory activities in medical records.
• Monitor patient responses to PT interventions and adjust as needed.
• Participate in care conferences, trainings, audits and quality activities.
• Maintain patient confidentiality and coordinate follow-up care with involved providers.
• Perform additional duties as assigned.
