Ascension at Home (together with Compassus) is seeking a Home Health Physical Therapist Assistant to work one-on-one with patients in their home, delivering skilled care and supporting independence in daily activities.

Minimum qualifications include an accredited PTA degree, state licensure, CPR, TB screen and hepatitis consent/declination, plus at least one year in home health. Comprehensive benefits and tuition assistance are provided.

Qualifications

• Bachelor or Associate Degree from an accredited PTA program.
• Currently licensed in state of practice.
• Current CPR, TB screen and Hepatitis consent/declination.

Responsibilities

• Provide one-on-one PTA care to patients in their home.
• Help patients regain strength and independence for daily living activities.
• Carry out duties per the home health plan of care.
