Job summary

Pinnacle Home Care is seeking a Full Time Physical Therapist Assistant (PTA) to provide in-home rehabilitation across Florida. Join a patient-centered team delivering care where it matters most – at patients’ homes.

Responsibilities include evaluating care plans, documenting progress, and guiding patients through therapeutic exercises. Florida PTA license and current BLS required; home health experience preferred and EMR familiarity a plus.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Florida PTA license with current BLS certification.
• Reliable transportation with valid driver’s license and insurance.
• One year of home healthcare experience preferred; new grads welcome.
• Experience with electronic medical records a plus.
• Strong communication and physical stamina.

Responsibilities

• Assist in evaluating and developing patient care plans and periodic reassessments.
• Document progress notes for PT review.
• Perform therapeutic exercises and assist patients with self-help devices.
• Instruct patients and families on therapy exercises alongside the PT.
• Monitor and report adverse drug reactions or ineffective therapies to the PT.
• Observe, document, and report clinical findings to the PT and physician as needed.
• Coordinate with PT and nursing staff on patient discharge plans.
• Educate health team members and families to support patient care.
• Maintain accurate documentation and comply with agency policies.
