Job summary

Pinnacle Home Care in Florida is seeking a Full Time Physical Therapist Assistant (PTA) to support patients in Highlands County and South Polk, including Lake Wales and nearby areas. You will work under the PT to implement care plans and help patients recover at home.

Ideal candidates hold a Florida PTA license, BLS, and have home health experience or are new grads ready to start. CEUs are offered to support licensure and growth.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Licensed PTA in Florida with current BLS certification.
• Experience with electronic medical records is a plus.
• Strong communication skills and reliability are essential.

Responsibilities

• Assist in evaluating and developing patient care plans and periodic reassessments.
• Document daily progress notes and PT reviews for submission.
• Perform therapeutic exercises and assist patients with self-help devices.
• Instruct patients and families on exercises alongside the PT.
• Monitor and report adverse reactions or ineffective therapies to the PT.
• Observe and report clinical findings to the PT and physician as needed.
• Coordinate discharge plans and summaries with PT and nursing staff.
• Educate health team members and families to support patient care.
