Pinnacle Home Care in Florida is seeking a Physical Therapist Assistant (PTA) for full-time and PRN positions to support patients in Clermont, Haines City, Davenport, Four Corners, Dundee and surrounding communities.

Qualifications

• Must be a graduate of an accredited PTA program.
• Active Florida PTA license with BLS certification is required.
• Home healthcare experience is preferred for this role.
• Experience with EMR systems is a plus.

Responsibilities

• Assist in evaluating and developing patient care plans and periodic reassessments.
• Document progress notes for PT review and submission.
• Perform therapeutic exercises and assist patients with self-help devices.
• Instruct patients and families on therapy exercises with the PT.
• Monitor and report adverse reactions or ineffective therapies to the PT.
• Observe, document, and report clinical findings to the PT and physician.
• Coordinate discharge plans and summaries with PT and nursing staff.
• Educate health team members and families to support patient care.
• Maintain accurate documentation and comply with agency policies.
