Job summary

Hartford HealthCare at Home in Connecticut seeks a licensed Physical Therapist Assistant to deliver PT treatments under supervision. You will assist with assessments, implement plans, document progress, and coordinate with the care team in patients across home care settings.

The role supports patient independence and quality of life with education for patients and families, while ensuring safety and compliance in home-based rehab services.

Qualifications

• Knowledge of principles and practices of Physical Therapy.
• Excellent communication and organizational skills.
• Excellent interpersonal skills; ability to interact with patients, families and co-workers.
• Ability to deal effectively with stress and ability to exercise independent judgment.

Responsibilities

• Assists the Physical Therapist with patient assessment (ROM, strength, mobility).
• Recognizes physiological/age-related changes; consults with PT.
• Under supervision, implements treatment plans and delivers patient care.
• Communicates patient progress to the Physical Therapist.
• Provides patient/caregiver education on treatment and equipment use.
