Job summary

One Home Health LLC seeks compassionate Physical Therapist Assistants (PTAs) to join our home health team across Illinois, including South Suburbs, Downtown Chicago, North Side Chicago, and Lake County. You will deliver one-on-one care in patients' homes and help them regain mobility and independence.

This flexible role offers full-time, part-time, or PRN options, competitive pay of $45–$55 per hour, and strong clinical support.

Qualifications

• Active Illinois PTA license.
• Graduate of an accredited PTA program recognized by the APTA.
• Minimum of six (6) months PTA experience.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Ability to travel to patient homes within assigned service areas.
• Ability to work independently while maintaining high standards of patient care.

Responsibilities

• Deliver physical therapy treatments and education to patients in their homes under the direction of a licensed Physical Therapist and according to the physician-approved plan of care.
• Assist patients in improving mobility, strength, balance, endurance, and functional independence.
• Observe, document, and report patient responses, progress, concerns, and changes in condition to the supervising Physical Therapist.
• Complete all required clinical documentation accurately and within agency timelines and compliance standards.
• Educate patients and caregivers on therapeutic exercises, fall prevention, home safety, and treatment goals.
• Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to coordinate patient care.
• Participate in case conferences, interdisciplinary meetings, and care planning discussions as needed.
• Follow infection prevention protocols, safety procedures, and equipment guidelines during patient visits.
• Maintain patient confidentiality and adhere to all agency, state, federal, and professional practice requirements.
• Attend staff development programs, training sessions, and quality improvement initiatives.
